I agree! Those questionnaires! I dislike them so much, for the reasons you mention. I get irate! I have just stopped filling them out and stopped trying to check in on line.
Funny thing is that when I arrive at an appointment, no one seems to care that I didn't fill out the form. At PT, no one has ever brought it up. At the sleep clinic, they were prepared and handed me the questionnaire on a clipboard. That's fine with me; I can add my editorial comments.
False information also shows up in the provider's notes. I think it is added automatically, as the same material appears in the notes of every appointment. It implies they did a much more thorough exam than what actually happened, reporting that, for example, the skin was good, when they never looked at my skin. I should ask someone about this. I have a care coordinator; she might know why this happens.
